All library authorities running the Summer Reading Challenge are required to complete this evaluation form.

Please note: This is the only library evaluation form this year. All authorities (including libraries taking part in the cross-authority model of delivery) should complete this form.

Please submit your evaluation form by email to [email protected] by 10 October 2024.

Send your evaluation form as an Excel spreadsheet and only submit one form per authority. We are unable to accept forms submitted as PDFs or Google Sheets.
